Going the Distance
When a patient living on the streets missed an appointment with his provider, our Medical Outreach Street Team knew the situation was urgent. He was living with a softball-sized hernia that required surgery and could not safely wait. Rather than losing him, the team searched areas they knew he frequented. Once the team found him, they assessed his condition and worked quickly to finish scheduling his surgery and secure a safe place for him to recover, coordinating every step.
On the day of his procedure, they transported him to the hospital, ensuring he arrived safely and on time. The team’s ability to adapt helped prevent a potentially life-threatening emergency and reduced the strain on local EMS and emergency departments, keeping resources available for other crises.
