LGBTQIA+ RESOURCES

Fourth Street Clinic welcomes, accepts and provides care for everyone, regardless of race, gender identity or sexual orientation:

  • WPATH mental health letters
 for surgery
  • Hormone therapy coordination
  • Gender affirming surgery referrals
  • Respite housing for surgery recovery
  • Medicaid enrollment services
  • Case Management assistance for legal name and gender change
 (for Utah residents)

Weekly Wellness Classes:

Making lifestyle changes to overcome chronic disease, addiction or mental illness requires knowledge, skills and long-term encouragement. You do not have to do it alone and you will have more success with support. Fourth Street Clinic offers several healthy lifestyle classes to give you the tools you need for success.

Wellness classes are free to attend. Talk to your provider to see if any of these classes are right for you.

Check-in time is 2:30pm. All classes are from 3pm – 4pm.

Below is a list of our current health education classes offered each week:

MONDAYS:

Seeking Safety A weekly group for women, non-binary and gender-non-conforming folks to learn present-focused coping skills to help increase safety. Each week will focus on a new skill for coping with the experience of trauma and/or substance use. Space is limited to eight participants.

TUESDAYS:

 

Diabetes Wellness & Education Learn about diabetes and how to manage self-care in areas such as meal planning, physical activity, medication and blood sugar. Learn from your peers and a diabetic specialist.

WEDNESDAYS:

Hep-C Learn the keys to successful treatment for Hepatitis C and get prepared for your first appointment with a treatment provider including lab work, immunizations and education. Space is limited. Appointments must be scheduled in advance with Fourth Street Clinic staff.

NEEDLE EXCHANGE
Hosted by the Utah Harm Reduction Coalition to help reduce the spread of diseases like HIV and Hepatitis. 3:00-5:00PM

THURSDAYS:

Come As You Are (CAYA) is an alternative to 12-step groups for anyone looking to change their relationship with substances. CAYA is the place to be if you are searching for a non-judgmental community for people who are in all stages of recovery and looking for affirmation, connection and validation. Space is limited to eight participants.

FRIDAYS:

Get Up and Move! Come learn how to improve your wellbeing through healthy eating tips, lifestyle choices and exercise routines.

Medically Vulnerable People (MVP) Program Referral Form
The MVP program provides interim housing and medical services to seniors, veterans and the medically frail who are experiencing homelessness. It provides a safe and supportive environment where individuals can connect with housing services, rehabilitate from illness, and access connections to the necessary health care and care management services with the goal to find stability and move toward permanent housing and other long-term solutions.
