As the climate on homelessness shifts dramatically in downtown Salt Lake City, it’s easy to forget that the complexities of homelessness haven’t changed. Access to health care services is crucial. It helps individuals receive ongoing treatment while getting back on their feet. The announcement of three new shelter sites and increased police presence around Pioneer Park have many wondering where individuals experiencing homelessness will receive health care.
Fourth Street Clinic is expanding, working with community partners and establishing a “Hub & Spoke” model of care. We’ll reach out to new patients both onsite with Nurse Care Managers and mobile medical units, taking care to those who need it first.
Phase 1 of the “spokes” includes placing a Registered Nurse at First Step House, YWCA-Utah, and the Housing Authority of Salt Lake County. The RN provides triaged care and chronic disease and medication management. Residents of these community partners often don’t receive ongoing treatment or essential health care services. The clinic remains downtown as the “hub” of homeless health care, and the mobile medical unit reaches the most vulnerable–building relationships and improving their overall health and quality of life.
With Phase 1 now fully established, Fourth Street Clinic treats clients at The Road Home and Volunteers of America Utah. This model of care will be implemented when the three new homeless resource centers go online, solidifying Fourth Street Clinic’s role as the health care provider for homeless Utahns.
