Our Partnerships Reduce Barriers to a Better Life
Fourth Street Clinic partners with shelters, resource centers and other community organizations throughout Salt Lake County to bring integrated health care directly to people experiencing homelessness. Through mobile health services and on-site clinical support, we meet patients where they are, reducing barriers to care and connecting them with ongoing medical, behavioral health and supportive services.
By bringing health care to our patients within community settings, Fourth Street Clinic helps individuals and families access primary care, manage chronic conditions and address urgent health needs. These partnerships strengthen the community’s coordinated response to homelessness by ensuring people receive compassionate, person-centered care when and where they need it most.
