Our partnerships reduce barriers to a better life

Fourth Street Clinic expanded its reach, working with community partners to establish a “Hub & Spoke” model of care. We reach out to new patients onsite with our Outreach services, taking care to those who need it first.

In 2019 Salt Lake County transitioned to a scattered site model for sheltering and serving individuals experiencing homelessness, Fourth Street Clinic responded by launching a Mobile Medical Clinic to bring primary care to clients at resource centers and community partners. Our Mobile Medical Clinic continues to be in demand to serve vulnerable patients at 11 locations.

The three new resource centers provide temporary accommodations with a high level of supportive services that will help individuals and families experiencing homelessness resolve their immediate housing crisis and swiftly secure permanent housing opportunities.

Each resource center provides not only a safe, warm place to sleep, but also crises services throughout the day including case management, employment services, meals, storage, housing navigation, and connections to long-term community support. Facilities are served by a mobile health clinic, and clients have access to a Forth Street Clinic EMT to help reduce mortality rates and minimize emergency services.
