Our partnerships reduce barriers to a better life
Fourth Street Clinic expanded its reach, working with community partners to establish a “Hub & Spoke” model of care. We reach out to new patients onsite with our Nurse Care Management and Mobile Health Care programs, taking care to those who need it first.
The goal of the three new resource centers is to provide temporary accommodations with a high level of supportive services that will help individuals and families experiencing homelessness resolve their immediate housing crisis and swiftly secure permanent housing opportunities. Each resource center will provide not only a safe, warm place to sleep, but also crises services throughout the day including case management, employment services, meals, storage, housing navigation, and connections to long-term community support. Facilities will be served by a mobile health clinic, and clients will have on-site access to a nurse manager–provided by Fourth Street Clinic.
