When you give during Give One Raise Two, you’ll be putting twice the care into the hands of homeless Utahns.

And we’ll be taking the care to those who are far from a medical home.

As the climate on homelessness shifts dramatically in downtown Salt Lake City, it’s easy to forget that the complexities of homelessness haven’t changed. Access to health care services is crucial, helping individuals receive ongoing treatment while getting back on their feet. The announcement of three new shelter sites and increased police presence around Pioneer Park have many wondering where individuals experiencing homelessness will receive health care.

Fourth Street Clinic is expanding to new territory, working closely with community partners to establish a “Hub & Spoke” model of care. We’ll reach out to new patients both onsite with Nurse Care Managers and mobile medical units, taking care to those who need it first.

Phase 1 of the “spokes” includes placing a Registered Nurse at First Step House, YWCA-Utah, and the Housing Authority of Salt Lake County; providing triaged care and chronic disease and medication management. Residents of these community partners often don’t receive ongoing treatment or essential health care services. The clinic will remain downtown as the “hub” of homeless health care, while the mobile medical unit reaches the most vulnerable – building relationships and improving their overall health and quality of life.

When Phase 1 is fully established, Fourth Street Clinic will include The Road Home and Volunteers of America Utah as spokes. This model will then be integrated into the three homeless shelter sites, solidifying Fourth Street Clinic’s role as the health care provider for homeless Utahns.

A financial gift to Fourth Street Clinic means that an uninsured, homeless Utahn can be vaccinated against a number of deadly diseases; be treated for chronic disease or pain – often gone untreated; and for the first time ever, be given a solid diagnosis and treatment plan. Your gift will not only will prevent surgeries, hospitalizations and deaths, it will provide a path to good health and housing.
