Weekly Wellness Classes:
Making lifestyle changes to overcome chronic disease, addiction or mental illness requires knowledge, skills and long-term encouragement. You do not have to do it alone and you will have more success with support. Fourth Street Clinic offers several healthy lifestyle classes to give you the tools you need for success.
Wellness classes are free to attend. Talk to your provider to see if any of these classes are right for you.
Check-in time is 2:30pm. All classes are from 3pm – 4pm.

THURSDAY:
Hep-C Learn the keys to successful treatment for Hepatitis C and get prepared for your first appointment with a treatment provider including lab work, immunizations and education. Space is limited. Appointments must be scheduled in advance with Fourth Street Clinic staff.
NEEDLE EXCHANGE
Hosted by the Utah Harm Reduction Coalition to help reduce the spread of diseases like HIV and Hepatitis. 3:00-5:00PM

Medically Vulnerable People (MVP) Program Referral Form
The MVP program provides interim housing and medical services to seniors, veterans and the medically frail who are experiencing homelessness. It provides a safe and supportive environment where individuals can connect with housing services, rehabilitate from illness, and access connections to the necessary health care and care management services with the goal to find stability and move toward permanent housing and other long-term solutions.
