The Behavioral Health Clinic provides individual and group therapy, medication management for mental health, and medically assisted substance use disorder treatment options for established Fourth Street Clinic patients.

The Dental Clinic provides preventative, restorative and urgent dental services for established Fourth Street Clinic patients.

To receive dental care at Fourth Street Clinic, patients must be established with a Fourth Street Clinic medical provider.

Services may include:

  • Dental Cleanings and Periodontal Care
  • Fillings and Other Restorative Treatment
  • Extractions
  • Denture Services
  • Urgent Dental Care
  • Oral Health Education

Insurance & Access

Fourth Street Clinic accepts Medicaid, Medicare, and private insurance, however, insurance is not required. No patient will be turned away due to inability to pay or insurance status.

Services are offered on a sliding fee scale. You may need to pay a nominal fee for services, based on household income and family size.
